The following forms are available as .pdf files for you to download and print. You will need Adobe Reader to open these files. Adobe Reader is free and can be downloaded at the Adobe website.

Please note the following:
1. The forms available on this page are the same ones you would receive if you requested them through the Camp Office. Please feel free to contact us if you have additional questions.
2. Online registration for summer camp is possible through the Registration page; however, because of the need to have hard copies of parental signatures, printing and mailing .pdf files from this page is preferred.

Resident Camp and Day Camp (information here)

Following registration, our office staff will mail you the following documents:

• Medical Form and Medical History Form (required for all campers)
• Medication Administration Release Form (required for any campers bringing medications to camp)
• Camper Personal History Form
• Coaching Period Request Form (Day Camp only)

Leader In Training Program

• 2008 Leader in Training Application
• LIT Reference Form (three written references are required with application)
• "Dear LIT Applicant and Parent" Letter
